IntroductionLarge ganglion cyst formation arising from the infrapatellar fat pad are quite rare and only a few are mentioned in the literature. Open excision in these cases ismandatory.Case presentationWe report the case of a large infrapatellar fat pad ganglion in a man of 37 years with a chronic discomfort in the knee Greek.The ganglion cyst is from the pad and had no infrapatellarfat intrasynovial extension. The final diagnosis was determined by magnetic resonance imaging of the knee, and the lesion was treated by surgery.
Conclusions: These lesions are asymptomatic in most cases, but are often misdiagnosed as a meniscal or ligament of the knee joint.
Today, the trend for treatment of such lesions is arthroscopic excision, but when there is a large ganglion, as in this case report, treatment should be thorough and open resection. This report is intended primarily but not exclusively for clinicians and radiologists.

The borders of the cyst will be marked then cut with a scalpel. The physician sutures the cut and applies a dressing. The patient must rest for at least thirty-minutes.
Aspiration has a few complications as the medical management for ganglion cysts; allergic reactions to cortisone injection and infection are just some of the common complications. Excision has some complications like rigidity in the surgical site and formation of the scar. Ganglion cysts reappear after excision in approximately 5-15% of the cases, typically because the entire cyst wasnt removed.
On the other hand, aspirated ganglion cysts fade away and cause no further complications in 27%-67% of the cases. If ever the cyst will reappear anytime, the doctor may again order a repeat aspiration procedure. Injection of corticosteroids right after the aspiration technique is beneficial than aspirating the fluid alone. However, excision is still considered to be more effective even though the patient will manifest some rigidity in the surgical site, It is normal to have scar formation many weeks from the procedure.
